Unify Community Health on Mission AvenueLocationUnify Community Health on Mission AvenueCitySpokaneEmployment duration:Part timeExempt Status:Non-exemptWhat you’ll do:Provide Tomando Control de su Salud/CDSMP classes for community members in assigned geographic areas.This position provides Chronic Disease Self-Management classes online for our patients across the State of Washington. Our Chronic Disease Self-Management Program is an evidence-based curriculum created by Stanford University and currently managed by Self-Management Resource Center.Workshops are designed to teach participants ways to better manage their chronic conditions. Some of the topics leaders will learn and teach are: weekly action plans, exercise, nutrition, communication skills, managing emotions, and problem solving. Group leaders will always co-facilitate with another leader. Training to become a certified CDSMP Group Leader is provided.Qualifications:Education: High School Diploma or GED.Experience: Experience with a chronic illness, or as a caregiver to a family member with chronic illness. Experience as a volunteer or information community resource.Licenses/Certificates/Registration: None.Knowledge/Skills/Abilities Required: Ability/desire to help others, comfortable with public speaking, and strong communication and interpersonal skills.Fluent in Spanish preferred, but not required.Availability to work evenings occasionally required.About YVFWCWe serve more than 181,000 patients across 25 medical clinics, 14 dental clinics, 10 pharmacies, and 64 program sites in two states.
