Social Worker will provide medical social services to patients in the home through physician’s orders and under the supervision of the Agency Management.Assesses clients social and emotional needs related to illness and/or injuryRespond to treatment and adjustment to care including the relationship of the medical and nursing requirements to the home situation, financial resources and availability of community resourcesProvides rehabilitative and supportive casework geared toward obtaining maximum benefits from medical careHelps clients utilize family and community resources through referrals or linking directly with community health and welfare agenciesWorks to achieve appropriate continuity of care to assure clients of ongoing services to meet their needsMaintains ongoing outreach with community agencies in order to identify agency services and act as liaison between Agency and the communityProvides in-service education on social work related topics as needed/requestedPrepares clinical records (initial evaluations, plans of care, progress notes, discharge summaries) and submits them for the clinical record within agency policyRecommends new policies and/or changes in current policies to improve quality of client services as well as efficiency and effectiveness of servicesProvides care as outlined in the patient’s plan of care and documents appropriately in the recordCommunicates with clients for visit schedulesProvides appropriate frequency of follow-up visitsIs knowledgeable of the agency’s QI goals and participates as directedParticipates in Professional Advisory Committee as requestedUnderstands and contributes to the financial management of casesCompletes other assignments and duties as requested and assigned to linesCreative thinker who embraces thought leadershipCapable of making challenging decisions and willing to take responsibility for them.Graduate from an accredited social work program with a Master’s degree in related field One (1) year experience in a healthcare setting (hospital, clinic, rehabilitation center, etc) with qualified supervisionExperience should be in direct service to clients and their families experiencing social, emotional and/or medical problems. Valid driver’s license and automobile insurance
